Community Aged Care Packages

DOTS Allied Health provides Occupational Therapy, Social Work and Dietetic Assessments and interventions, funded via Community Aged Care packages.

The new flexibility of Community Aged Care Packages allows users to utilise their funding to meet their needs. 

As a fully mobile service, OT, Social Work and Dietetics are provided in-home. They can also be accessed from one of our clinics (Victoria Point and Capalaba). 

Occupational Therapy will typically assist with home modifications such as:
  • ​Rails next to the toilet
  • Shower rails
  • Step modifications
  • Access ramps
  • Bannister rails for stairs
  • ​Rails for safe exit and entry at doorways. 
  • Falls prevention measures
  • Other modifications
OT can also assist with assessment for mobility aids, including specialised wheelchairs and mobility scooters (and safe use); personal medical alarms (including those with GPS tracking for people with memory loss / confusion) and more. 

Dietetics
As we age, it is important to understand the impact of aging on our capacity for nutritional uptake as well as the additional nutritional considerations. 
Our dietitians are highly experienced and understand the requirement for healthy aging. They also have capacity to assist where disease processes impact nutrition, including the need for tube feeding and palliative care. 

Social Work  
In the context of Community Aged Care Packages, Social Work may be used to advocate for change, assist with change transition (such as moving house, moving to care, or managing changes in the family including grief) or promote community engagement. 
 
Through circumstance, many older people become socially isolated and benefit from support to find ways to re-engage in activities and socialising. Additionally, Social Work can assist with complex case management (assessment, planning, advocacy, linking with other services).
